The hard part of public health is rarely the discovery. Clean water, vaccines, and oral rehydration were all understood long before they reached everybody. The work is logistics: cold chains, supply, trained staff, and trust. Most of the gain in human life expectancy came from delivery, not from breakthroughs.
- In 1854 John Snow mapped cholera deaths around one water pump in London and had the handle taken off. Careful data, then one very small physical act.
- A vaccine sitting in a warehouse saves nobody. Refrigeration, roads, and record-keeping decide the outcome as much as the science that produced it.
